Community Help Center Case Manager

POSITION SUMMARY: Maintain the Community Help Center at our Grand Island office. Project the importance of the CHC and the impact the CHC has on the human service support services in the Grand Island area. Promote the CHC and CNCAP. Provide a variety of support services to families and individuals; provide assistance and referrals; identify and access client resources; establish and maintain public relations contacts, and secure donations from businesses and the public.

 

ROLE AND RESPONSIBILITIES

  1. Responsible for completing agency intake form and inputting it into Clarity
  2. Provide ongoing support and document families progress toward goals
  3. Maintain a non-judgmental attitude toward clients and other CNCAP staff
  4. Ensure clients are assessed through the application process to determine possible assistance through resources
  5. Create family personal plans to develop short and long term attainable goals
  6. Ensure files are complete with the necessary paperwork for clients participating in the program
  7. Adhere to all CNCAP related policies and procedures
  8. Promote the program and agency in a positive and professional manner

 

QUALIFICATIONS

  1. High school diploma or GED required (Bachelor's degree in human services or related field preferred)
  2. Valid driver's license
  3. 1+ years of experience in human services or related field

 

SKILLS & ABILITIES

  1. Problem-solving skills
  2. Communication skills
  3. Extensive knowledge in word, excel, powerpoint, and databases
  4. Excellent time-management and organizational skills
  5. Demonstrated ability to work with others

 

SCHEDULE: Monday-Thursday: 7am - 5pm or Monday-Friday: 8am - 4pm

REPORTS TO: Housing Programs Coordinator

JOB STATUS: Full-Time, 40 hours per week

PAY: $13.25/Hour
